INDUSTRIES CASE MANAGER
TradeWinds Services, located in Merrillville, IN, is a non-profit organization serving individuals with diverse abilities for over 50 years. We have a part-time position for a Case Manager in the Pre-Vocational department. Hours: Must be available Monday-Friday, 7:00 am - 3:30 pm.
Description of Position: Responsible for coordinating agency services that enable an individual with intellectual disabilities to receive a full range of appropriate services in a planned, coordinated, efficient and effective manner to promote the well-being of the individual. Responsible for locating, managing, coordinating, and monitoring all proposed services. Responsible for making recommendations for needed medical, social, educational, and informal community supports needed by eligible person within the day Services Program at TradeWinds. Responsible for oversight of funding resources to assure they are adequate for planned services proved by TradeWinds. Completion of weekly, monthly and annual paperwork to ensure TradeWinds remains in compliance.
Education/Certification Requirements: Bachelor’s Degree preferred
Experience Required: Must have one year of experience working directly with persons with intellectual disabilities. Prior experience in developing and implementing individual programs, program evaluation, and in working with a team is desired.
Additional Requirements: Must be at least 21 years of age and have reliable transportation with a valid driver’s license with no suspensions for traffic infractions during the past three years. Must be able to pass drug screening and criminal background checks.
